 If booking online and paying in person, fill out the calendar form and leave the bottom portion (Name, Phone Number, Price Packages) blank.

If paying online fill out both the calendar portion and the bottom portion (Name, Phone Number, Price Packages) to make sure you do not have to wait for the detail to be done and pay once completed.
